A Anthropometrics & Demographics
Age, Height & Weight Used for siMS Risk Score (Age/50 factor), IDRS scoring, Gurka Z-score, WHtR, BMI, and ICMR-NIN macronutrient gram targets.
Waist Circumference (cm) IDF 2005 mandatory criterion. Measure at the midpoint between lower rib and iliac crest at end of normal expiration. South Asian / Indian women: threshold ≥80 cm.
B Biochemical Markers (IDF Criteria 1–4)
Fasting Triglycerides (mg/dL) IDF threshold: ≥150 mg/dL. Used in siMS and Gurka Z-score. Omit value and tick treatment if on medication.
HDL Cholesterol (mg/dL) IDF threshold for women: <50 mg/dL. Also in siMS and Gurka formulas. Female-specific criterion (men: <40 mg/dL).
Blood Pressure (mmHg) IDF threshold: SBP ≥130 OR DBP ≥85 mmHg. Systolic BP used in siMS and Gurka formulas.
Fasting Plasma Glucose (mg/dL) IDF threshold: ≥100 mg/dL. Represents beta-cell failure to compensate peripheral insulin resistance. RSSDI: compute HbA1c annually if ≥100 mg/dL.
C Indian Diabetes Risk Score (Non-Invasive)
Physical Activity Level IDRS (MDRF) scoring. RSSDI 2022 target: ≥150 min/week moderate aerobic activity; up to 300 min/week post-menopausal.
Family History of Diabetes IDRS scoring. Also used as ×1.2 multiplier in siMS Risk Score if cardiovascular / cerebrovascular family history is also present.
Family History of CVD / Stroke Applies ×1.2 multiplier to siMS Risk Score if first-degree relative had premature cardiovascular or cerebrovascular event (Soldatovic 2016).
D Female-Specific Clinical Modifiers
Female-Specific Endocrine Status (select all that apply) PCOS: 45.7% have full MetSyn. GDM: 52.2% develop T2DM within 10 years. Menopause: visceral fat doubles (5–8% → 10–15% of body weight).
E Dietary Pattern & Racial Group
Dietary Pattern Determines food-based nutrition recommendations. ICMR-NIN 2024 My Plate approach with ethnic-specific priorities.
Racial / Ethnic Group for Gurka Z-Score Gurka MetS severity Z-score uses race/sex-specific equations (PMC4071942). South Asian / Indian: use Non-Hispanic White as closest validated proxy.
