Smoking status Current = any tobacco in past 30 days. Men +4 pts, Women +3 pts. In the FRS model, smoking points are removed once the patient is no longer a current smoker; real-world cardiovascular risk then declines progressively after cessation.

Known ASCVD / previous cardiovascular event? Prior MI, stroke, TIA, PCI, CABG, or diagnosed coronary / peripheral artery disease. If Yes, this is secondary prevention: the patient is already Very High Risk and the Framingham 10-year (primary-prevention) score will not be shown.

South Asian / Indian ethnicity? Applies a 1.4× risk multiplier for South Asian men (UK NICE/JBS clinical guidance). No validated multiplier exists for South Asian women; the results panel will explain why instead of applying an unvalidated number.

Family history of premature CVD? First-degree relative (parent, sibling, child) with MI/stroke before age 55 (men) or 65 (women). Reported as a risk-enhancing factor — it is not added to the Framingham score.

Waist Circumference (cm) (optional) Flags abdominal obesity at IDF South Asian cutoffs (♂ ≥90 cm · ♀ ≥80 cm), independent of BMI. Reported as a risk enhancer — not added to the Framingham score.

LDL-C (Direct / Calculated) mg/dL ICMR: optimal <100 mg/dL (<2.6 mmol/L). If LDL-C >190 mg/dL consider familial hypercholesterolaemia.

Triglycerides (TG / TRIG) mg/dL AHA: normal <150 mg/dL; borderline 150-199; high 200-499; very high >=500. High TG with low HDL = metabolic dyslipidaemia pattern common in South Asians.

Non-HDL-C (auto-calculated if TC + HDL entered above) mg/dL Non-HDL-C = TC minus HDL-C. Targets (CCS 2017): Low risk <4.2 mmol/L; High risk <2.6 mmol/L. Superior predictor to LDL-C alone in Indian metabolic phenotype.

HbA1c (Glycated Haemoglobin) % ADA criteria: normal <5.7%; pre-diabetes 5.7-6.4%; diabetes >=6.5%. Note: diabetes status for FRS scoring must still be confirmed above.

Fasting Plasma Glucose (FPG) mg/dL ADA: normal <100 mg/dL; IFG 100-125; diabetes >=126. WHO Indian reference: >=126 mg/dL on 2 occasions = diabetes.

Serum Creatinine mg/dL Used to estimate eGFR (CKD-EPI 2021). eGFR <60 mL/min/1.73m2 = CKD, which independently elevates CVD risk and modifies statin dosing.
