Smoking status Current = any tobacco in past 30 days. Men +4 pts, Women +3 pts. In the FRS model, smoking points are removed once the patient is no longer a current smoker; real-world cardiovascular risk then declines progressively after cessation.
Known ASCVD / previous cardiovascular event? Prior MI, stroke, TIA, PCI, CABG, or diagnosed coronary / peripheral artery disease. If Yes, this is secondary prevention: the patient is already Very High Risk and the Framingham 10-year (primary-prevention) score will not be shown.
South Asian / Indian ethnicity? Applies a 1.4× risk multiplier for South Asian men (UK NICE/JBS clinical guidance). No validated multiplier exists for South Asian women; the results panel will explain why instead of applying an unvalidated number.
Family history of premature CVD? First-degree relative (parent, sibling, child) with MI/stroke before age 55 (men) or 65 (women). Reported as a risk-enhancing factor — it is not added to the Framingham score.
Waist Circumference (cm) (optional) Flags abdominal obesity at IDF South Asian cutoffs (♂ ≥90 cm · ♀ ≥80 cm), independent of BMI. Reported as a risk enhancer — not added to the Framingham score.
